Piramal TS

Piramal TS (distributed by Emzor Pharmaceutical Industries) is an Artemisinin-based Combination Therapy (ACT) antimalarial specifically formulated to treat acute uncomplicated malaria and highly resistant Plasmodium strains. It combines two synergistic active pharmaceutical ingredients: dihydroartemisinin (DHA) and piperaquine phosphate.

• Dihydroartemisinin (120 mg): The main active metabolite of artemisinin derivatives. It acts rapidly against blood-stage trophozoites by generating intra-parasitic free radicals in the presence of heme iron, driving fast parasite clearance and symptom relief.

• Piperaquine Phosphate (960 mg): A long-acting bisquinoline antimalarial derivative structurally related to chloroquine. It accumulates within the parasite’s acidic food vacuole and inhibits the conversion of toxic free heme into inert hemozoin. The resulting accumulation of toxic heme destroys parasitic cell structures, providing prolonged clearance to prevent recrudescence.

Because of its high-strength fixed combination, a full adult therapeutic course requires a once-daily single-tablet administration for three consecutive days.

Primary Medical Indications

Piramal TS is indicated for adults and adolescents for the treatment of acute parasitic infections:

• Uncomplicated Plasmodium falciparum Malaria: First-line curative therapy for acute, uncomplicated malaria caused by P. falciparum or mixed infections containing P. falciparum.

• Multi-Drug Resistant Strains: Highly effective against parasitic strains exhibiting high resistance to traditional monotherapies (such as chloroquine or sulfadoxine-pyrimethamine).

• Erythrocytic Blood-Stage Infections: Clears blood-stage schizonts across sensitive Plasmodium species (P. falciparum, P. vivax, P. ovale, P. malariae). (Radical cure for dormant liver hypnozoites in P. vivax/ovale requires adjunct therapy with primaquine or tafenoquine).

Critical Safety Guidelines & Precautions

• Empty Stomach Requirement: High-fat meals significantly increase the bioavailability of piperaquine, which can elevate systemic plasma levels and increase the risk of cardiac adverse effects. Strictly follow the 3-hour pre- and post-dose fasting rule.

• QTc Interval Prolongation: Piperaquine carries a risk of QTc prolongation on an ECG. Avoid concurrent use with other QT-prolonging drugs (e.g., macrolide antibiotics, antiarrhythmics, quinine) or in patients with uncorrected electrolyte imbalances or congenital long QT syndrome.

• Transient Adverse Effects: Mild dizziness, headache, nausea, vomiting, or abdominal discomfort may occur. Patients experiencing dizziness or vertigo should avoid driving or operating heavy machinery.

• Re-Dosing for Vomiting: If vomiting occurs within 30 to 60 minutes of taking a dose, re-administer a full replacement dose. If persistent vomiting prevents oral retention, switch to parenteral antimalarial therapy (e.g., IV or IM artesunate).

• Not for Severe/Complicated Malaria: Piramal TS is an oral formulation intended for uncomplicated malaria. Severe, complicated, or cerebral malaria requires immediate emergency hospitalization and intravenous artesunate administration.

• Prophylaxis Restriction: Piramal TS is strictly a therapeutic curative agent and must never be used for malaria prophylaxis.
