Swidar (manufactured by Swiss Pharma Nigeria Ltd / Swipha) is a widely recognized fixed-dose combination antimalarial formulation. It belongs to the antifolate class of antiparasitic medications and is composed of two synergistic active ingredients: sulfadoxine and pyrimethamine.

Sulfadoxine (500 mg): A long-acting sulfonamide antibacterial/antiprotozoal agent. It competitively inhibits the parasite enzyme dihydropteroate synthase (DHPS), blocking the initial step of folate synthesis in Plasmodium parasites.

Pyrimethamine (25 mg): An aminopyrimidine derivative. It selectively inhibits dihydrofolate reductase (DHFR), stopping the subsequent conversion of dihydrofolic acid to tetrahydrofolic acid.
By targeting two sequential steps in the parasite’s essential folic acid biosynthesis pathway, the combination produces a potent synergistic lethal effect against the asexual blood stages of Plasmodium falciparum and other sensitive species.
Primary Medical Indications
Swidar is indicated for targeted curative and preventive malaria interventions:

Intermittent Preventive Treatment in Pregnancy (IPTp-SP): Used routinely during antenatal care in endemic regions to prevent maternal anemia, low birth weight, and placental malaria. Recommended as monthly single-dose treatments starting in the second trimester up until delivery.

Perennial & Seasonal Malaria Chemoprevention (PMC / SMC): Administered in specific epidemiological settings to infants or young children to reduce seasonal malaria burden.

Treatment of Acute Uncomplicated Malaria: Indicated as a single-dose treatment for acute P. falciparum malaria. In modern clinical guidelines, it is frequently co-administered with an artemisinin derivative (e.g., artesunate) as part of an Artemisinin-based Combination Therapy (ACT) to reduce the risk of drug resistance.

Sulfonamide Hypersensitivity: Strictly contraindicated in individuals with known allergies to sulfa drugs, sulfadoxine, or pyrimethamine. Severe cutaneous reactions (such as Stevens-Johnson syndrome or toxic epidermal necrolysis) can occur. Discontinue immediately at the first sign of skin rash.
