Active Ingredients & Dual Mechanism of Action

1. Artemether: A semi-synthetic derivative of artemisinin that acts rapidly against the erythrocytic (blood) stages of the malaria parasite. It generates reactive oxygen radicals that disrupt parasite cellular membranes, resulting in swift parasite clearance and rapid fever relief.
2. Lumefantrine: A lipophilic aryl-alcohol antimalarial that acts more slowly but stays in the body longer. It inhibits the detoxification of toxic heme (a byproduct of hemoglobin breakdown) into non-toxic hemozoin within the parasite’s food vacuole, leading to parasite cell destruction.

Dosage Forms & Available Strengths

• Lokmal QS Caplets / Tablets (High Dose): Contains 80 mg Artemether / 480 mg Lumefantrine per caplet (standard 6-caplet pack for adult convenience).
• Lokmal Standard Tablets: Contains 20 mg Artemether / 120 mg Lumefantrine per tablet (standard 24-tablet blister pack).
• Lokmal Dispersible Tablets: Pediatric formulation designed to dissolve in a small amount of water for children.
• Lokmal Oral Suspension: Liquid form for infants and young children.

Clinical Indications

• Uncomplicated Malaria: Treatment of acute, uncomplicated malaria infections caused by Plasmodium falciparum or mixed Plasmodium infections.
• Note: Lokmal is not used to prevent malaria (prophylaxis) and is not indicated for severe or complicated cerebral malaria requiring intravenous therapy.

Key Pharmacokinetics

• Absorption: Both components are absorbed orally. Lumefantrine absorption is significantly enhanced (up to 16-fold) when taken with fatty food or milk.
• Peak Levels: Artemether reaches peak plasma concentration within 2 hours; lumefantrine reaches peak levels in about 6 to 8 hours.
• Half-Life: Artemether has a short elimination half-life ($\sim 2\text{ hours}$), whereas lumefantrine has a prolonged half-life ($\sim 3\text{ to }6\text{ days}$), clearing remaining parasites.

Administration Guidelines

• Food Requirement: Must be taken with meals or drinks containing fat (such as whole milk, soup, or oil-containing food) to ensure full absorption of lumefantrine.
• Standard 3-Day Schedule (6 Doses Total):
  • Initial Dose: Take the first dose immediately upon diagnosis.
  • Second Dose: Take 8 hours after the first dose.
  • Days 2 & 3: Take one dose in the morning and one dose in the evening (12 hours apart) for two consecutive days.
• Vomiting Management: If vomiting occurs within 1 hour of taking a dose, a full replacement dose should be taken immediately.

Safety Profile & Considerations

• Common Side Effects: Headaches, dizziness, loss of appetite, fatigue, nausea, abdominal pain, joint/muscle pain, and sleep disturbances.
• Precautions & Cardiac Risk: Artemether/lumefantrine can cause prolongation of the QT interval on an ECG. Caution is advised in patients with pre-existing heart rhythm conditions, severe hypokalemia, or those taking other QT-prolonging medications.
• Pregnancy & Lactation: Caution is advised during the first trimester of pregnancy; it may be prescribed in the second and third trimesters if clinically indicated and supervised by a doctor.
• Key Interactions: Avoid co-administration with strong CYP3A4 inducers (such as rifampicin, carbamazepine, or St. John’s Wort) as they significantly lower blood levels of the antimalarial active agents.

Note: Lokmal is a prescription antimalarial. Full completion of the entire 3-day treatment course is critical to prevent treatment failure and parasite resistance, even if symptoms improve quickly.
