Exforge is a brand-name fixed-dose combination prescription drug developed by Novartis for the management of essential hypertension (high blood pressure). It combines two complementary blood pressure-lowering agents—Amlodipine (a calcium channel blocker) and Valsartan (an angiotensin II receptor blocker)—into a single tablet.
In certain global markets, a triple-combination variant known as Exforge HCT is also available, which adds Hydrochlorothiazide (a thiazide diuretic) to the amlodipine and valsartan regimen.
Active Ingredients & Dual Mechanism of Action
Exforge targets two key vascular pathways to achieve comprehensive 24-hour blood pressure control:

Amlodipine Besylate (CCB): Inhibits the entry of extracellular calcium ions into vascular smooth muscle and cardiac cells. This promotes direct relaxation and dilation of peripheral blood vessels, reducing total peripheral vascular resistance.

Synergistic Advantage: The combination delivers superior blood pressure reduction compared to either drug taken alone at the same dosage. Additionally, the valsartan component helps offset peripheral ankle edema (swelling) commonly triggered by amlodipine.

Boxed Warning (Fetal Toxicity): Drugs acting on the renin-angiotensin system can cause severe injury or death to the developing fetus. Discontinue Exforge immediately if pregnancy is detected.

Contraindications: Known hypersensitivity to amlodipine, valsartan, or other dihydropyridines. Do not co-administer with aliskiren in patients with diabetes. Avoid in patients with severe hepatic impairment, biliary obstruction, or severe renal failure.
