Cefuroxime

Cefuroxime is a versatile, second-generation cephalosporin antibiotic available in both oral (cefuroxime axetil) and parenteral (cefuroxime sodium) formulations. It exerts a bactericidal effect by binding to essential penicillin-binding proteins (PBPs) in the bacterial cell wall, inhibiting peptidoglycan synthesis and causing bacterial cell lysis. Cefuroxime exhibits strong resistance to degradation by many bacterial beta-lactamase enzymes, giving it a broad spectrum of activity against both Gram-positive and Gram-negative pathogens, including Streptococcus pneumoniae, Haemophilus influenzae, and Moraxella catarrhalis.

Primary Medical Indications

Cefuroxime is prescribed across adult and pediatric populations for mild, moderate, and severe bacterial infections, including:

• Lower & Upper Respiratory Infections: Community-acquired pneumonia (CAP), acute bronchitis, acute exacerbations of chronic obstructive pulmonary disease (COPD), pharyngitis, and tonsillitis.

• ENT Infections: Acute bacterial otitis media (middle ear infection) and acute bacterial sinusitis.

• Urinary Tract Infections (UTIs): Uncomplicated and complicated cystitis, pyelonephritis, and urethritis.

• Skin & Soft Tissue Infections: Uncomplicated skin infections, cellulitis, impetigo, and wound infections.

• Lyme Disease: Treatment of early-stage Lyme disease (erythema migrans) in adults and children over 12 years.

• Surgical Prophylaxis: Parenteral administration (cefuroxime sodium) prior to cardiac, orthopedic, or abdominal surgeries to reduce post-operative infection risks.

Important Safety & Clinical Precautions

• Penicillin & Cephalosporin Allergy: Exercise caution in patients with a history of penicillin allergy due to potential cross-reactivity. Strictly contraindicated in patients with a history of severe immediate hypersensitivity (anaphylaxis) to any cephalosporin or beta-lactam antibiotic.

• Food Requirement for Oral Tablets: Gastrointestinal absorption of oral cefuroxime axetil tablets is significantly improved when administered immediately following a meal.

• Stomach Acid Reducers: Concurrent use of H2-receptor antagonists, proton pump inhibitors (PPIs), or antacids can reduce gastric acidity, decreasing the bioavailability and efficacy of oral cefuroxime.

• Bacterial Specificity: Cefuroxime treats bacterial infections only and has no activity against viral infections like influenza, the common cold, or COVID-19.
