Virest is a brand-name oral antiviral medication containing Aciclovir (also spelled Acyclovir) as its active ingredient. It belongs to the synthetic nucleoside analog class of antivirals and is primarily used to manage viral infections caused by the herpes virus family.

Active Ingredient & Mechanism of Action

  • Active Ingredient: Aciclovir (Acyclovir).
  • Mechanism: Aciclovir is converted intracellularly by viral thymidine kinase into its active form, aciclovir triphosphate. This active metabolite selectively inhibits viral DNA polymerase and incorporates into growing viral DNA chains, causing immediate termination of viral replication without disrupting healthy human cellular processes.

Available Formulations & Strengths

Virest is supplied in oral solid dosage forms:
  • Virest 200 mg Tablets: Lower-strength oral tablets.
  • Virest 400 mg Tablets: Standard oral tablets.

Clinical Indications

Virest (Aciclovir) is prescribed for:
  • Herpes Simplex Virus (HSV) Types 1 & 2: Treatment of primary and recurrent genital herpes, as well as oral herpes (cold sores).
  • Herpes Zoster (Shingles): Acute treatment of localized or disseminated shingles outbreaks to accelerate lesion healing and reduce severity/duration of nerve pain.
  • Varicella (Chickenpox): Management of active chickenpox infections, particularly in adolescents, adults, or high-risk populations.
  • Suppressive Therapy: Chronic daily administration to prevent or minimize frequent, recurrent genital herpes outbreaks in infected individuals.
Note: Aciclovir controls viral replication and speeds healing, but it does not completely eradicate the latent virus from the body (it is not a cure).

Key Pharmacokinetics

  • Absorption: Modest oral bioavailability ($\sim 15\text{–}30\%$), which is unaffected by food intake.
  • Distribution: Distributes widely into body tissues and fluids, including vesicular fluid, brain tissue, and cerebrospinal fluid (CSF).
  • Elimination & Half-Life: Elimination half-life is approximately 2.5 to 3 hours in adults with normal renal function. Excreted predominantly unchanged through the kidneys into the urine via glomerular filtration and tubular secretion.

Administration Guidelines

  • Hydration (Crucial): Patients taking Virest must drink plenty of fluids throughout the day to prevent aciclovir crystals from precipitating in the renal tubules.
  • Timing: Can be taken with or without food. Doses should be spaced evenly throughout waking hours as directed by a healthcare provider.
  • Early Initiation: Treatment yields the highest efficacy when started at the very first sign or symptom (tingling, burning, or initial appearance of lesions) of an outbreak.

Safety Profile & Considerations

  • Common Side Effects: Headache, nausea, vomiting, diarrhea, abdominal pain, dizziness, fatigue, and mild skin rash.
  • Key Precautions & Warnings:
    • Renal Impairment: Because aciclovir is eliminated by the kidneys, dose reductions and extended dosing intervals are mandatory for patients with reduced renal function or dehydration.
    • Neurotoxicity: Rare neurological events (confusion, agitation, hallucinations, tremors, or drowsiness) can occur, most commonly in elderly patients or those with underlying kidney impairment.
  • Contraindications: Known hypersensitivity to aciclovir, valaciclovir, or any formulation components.
  • Key Drug Interactions:
    • Probenecid: Decreases renal clearance of aciclovir, elevating its plasma levels and half-life.
    • Other Nephrotoxic Drugs: Co-administration with agents like aminoglycosides, amphotericin B, or NSAIDs increases the risk of renal toxicity.
Note: Virest is a prescription-only antiviral medication. Dosage schedules, duration of therapy, and renal monitoring must be supervised by a licensed healthcare professional.
