Active Ingredient & Mechanism of Action

• Active Ingredient: Aciclovir (Acyclovir).

Clinical Indications

• Herpes Simplex Virus (HSV) Types 1 & 2: Treatment of primary and recurrent genital herpes, as well as oral herpes (cold sores).
• Herpes Zoster (Shingles): Acute treatment of localized or disseminated shingles outbreaks to accelerate lesion healing and reduce severity/duration of nerve pain.
• Varicella (Chickenpox): Management of active chickenpox infections, particularly in adolescents, adults, or high-risk populations.
• Suppressive Therapy: Chronic daily administration to prevent or minimize frequent, recurrent genital herpes outbreaks in infected individuals.

Key Pharmacokinetics

• Absorption: Modest oral bioavailability ($\sim 15\text{–}30\%$), which is unaffected by food intake.
• Distribution: Distributes widely into body tissues and fluids, including vesicular fluid, brain tissue, and cerebrospinal fluid (CSF).
• Elimination & Half-Life: Elimination half-life is approximately 2.5 to 3 hours in adults with normal renal function. Excreted predominantly unchanged through the kidneys into the urine via glomerular filtration and tubular secretion.

Administration Guidelines

• Hydration (Crucial): Patients taking Virest must drink plenty of fluids throughout the day to prevent aciclovir crystals from precipitating in the renal tubules.
• Timing: Can be taken with or without food. Doses should be spaced evenly throughout waking hours as directed by a healthcare provider.
• Early Initiation: Treatment yields the highest efficacy when started at the very first sign or symptom (tingling, burning, or initial appearance of lesions) of an outbreak.
