Active Ingredient & Mechanism of Action

• Active Ingredient: Mebendazole.
• Mechanism: Mebendazole acts locally in the gut by selectively binding to the $\beta$-tubulin of susceptible parasitic worms:
  1. Inhibition of Microtubule Synthesis: It disrupts cytoplasmic microtubule formation in the parasite’s intestinal cells.
  2. Irreversible Energy Depletion: This leads to the blocking of glucose uptake and nutrient absorption, gradually causing the worm to lose energy, die, and be passed naturally in the stool.
• Selectivity: Mebendazole has a much higher affinity for parasite $\beta$-tubulin than for human tubulin, making it safe for host tissues at standard therapeutic doses.

Dosage Forms & Available Strengths

• Chewable Tablets: Standard 100 mg chewable tablets (often orange-flavored).
• High-Dose / Single-Dose Tablets: 500 mg chewable tablets (frequently used for mass treatment programs or specific infections).
• Oral Suspension: 100 mg / 5 mL liquid suspension for young children unable to chew tablets.

• Enterobiasis (Pinworm / Threadworm): Enterobius vermicularis
• Ascariasis (Large Roundworm): Ascaris lumbricoides
• Trichuriasis (Whipworm): Trichuris trichiura
• Hookworm Infections: Ancylostoma duodenale and Necator americanus

Key Pharmacokinetics

• Absorption: Poorly absorbed from the gastrointestinal tract ($< 10\%$ reaches systemic circulation). This low bioavailability keeps the active drug localized inside the intestinal lumen where the parasites reside.
• Metabolism & Elimination: The small portion that is systemically absorbed undergoes extensive first-pass metabolism in the liver. It is excreted mainly in feces (unchanged and metabolized) with less than 2% excreted in urine.

Administration Guidelines

• For Pinworms (Threadworms):
  • Adults & Children over 2 years: 100 mg single dose. Because reinfection is very common, a second dose of 100 mg is recommended 2 to 3 weeks later. Treating the entire household simultaneously is often advised.
• For Roundworms, Whipworms, & Hookworms:
  • Adults & Children over 2 years: 100 mg twice daily (morning and evening) for 3 consecutive days (total of 600 mg).
• Method: Tablets may be chewed completely, swallowed whole with water, or crushed and mixed with food for children. No special purging, fasting, or laxatives are required.

Safety Profile & Considerations

• Common Side Effects: Generally very well tolerated due to minimal systemic absorption. Transient side effects may include mild abdominal pain, diarrhea, flatulence, and dizziness (often due to the clearance of dying worms from the gut).
• Age Limits: Generally not recommended for infants under 1 year of age due to limited safety data and reports of convulsions in this age group.
• Pregnancy & Lactation: Should be avoided during the first trimester of pregnancy unless explicitly advised by a physician based on clinical necessity.
• Key Drug Interactions:
  • Cimetidine: May inhibit hepatic metabolism of mebendazole, increasing plasma levels.
  • Metronidazole: Concurrent high-dose use has been associated with rare reports of severe skin reactions (Stevens-Johnson syndrome / toxic epidermal necrolysis); co-administration is generally avoided.
