T-Beta

Active Ingredients & Dual Mechanism of Action

1. Travoprost (0.004% w/v): A synthetic prostaglandin F2$\alpha$ analog.
   • Mechanism: Increases the fluid drainage out of the eye primarily via the uveoscleral pathway (and secondarily through the trabecular meshwork).
2. Timolol Maleate (0.5% w/v): A non-selective beta-adrenergic receptor blocker.
   • Mechanism: Decreases the production of aqueous humor (fluid) by the ciliary body inside the eye.

Dual Effect: One component increases fluid outflow (Travoprost) while the other decreases fluid inflow (Timolol), effectively reducing intraocular pressure through two independent pathways.

Clinical Indications

• Open-Angle Glaucoma: To slow or prevent progressive optic nerve damage and vision loss caused by elevated eye pressure.
• Ocular Hypertension: To reduce dangerously high fluid pressure inside the eye before structural nerve damage occurs.
• Insufficient Response to Monotherapy: Prescribed when single-agent eye drops (prostaglandins alone or beta-blockers alone) fail to achieve the target intraocular pressure.

Administration Guidelines

• Standard Dosing: Instill 1 drop in the affected eye(s) once daily, preferably in the morning (or as specifically directed by an ophthalmologist).
• Application Technique:
  • Wash hands thoroughly. Tilt head back and pull down the lower eyelid to form a small pocket.
  • Squeeze 1 drop into the eye without touching the dropper tip to the eye, eyelid, or fingers.
  • Nasolacrimal Occlusion (Crucial Step): Close the eye gently and press a finger against the inner corner of the eye (near the nose) for 1 to 2 minutes. This prevents the medication from draining into the tearduct and entering systemic circulation, significantly reducing systemic side effects.
• Contact Lenses: Remove contact lenses prior to administration. Wait at least 15 minutes before reinserting them, as preservatives (such as benzalkonium chloride) can be absorbed by soft lenses.
• Multiple Eye Drops: If using other ophthalmic drops, wait at least 5 minutes between instilling different medications.

Safety Profile & Critical Warnings

• Contraindications (Do Not Use If You Have):
  • Reactive Airway Diseases: Asthma, severe Chronic Obstructive Pulmonary Disease (COPD), or a history of bronchospasm. (Beta-blockers can cause fatal bronchospasm).
  • Cardiovascular Conditions: Sinus bradycardia (very slow heart rate), second- or third-degree atrioventricular (AV) block, overt cardiac failure, or cardiogenic shock.
• Local Eye Side Effects:
  • Ocular redness (hyperemia), mild stinging, burning, or dry eye sensation.
  • Prostaglandin-Induced Changes: Long-term use can cause permanent increased brown pigmentation of the iris, darkening of eyelid skin, and increased growth/thickening of eyelashes.
• Systemic Side Effects: Slow heart rate (bradycardia), low blood pressure (hypotension), dizziness, fatigue, and exacerbation of asthma symptoms.
