Active Ingredient & Dual Mechanism of Action

• Active Ingredient: Olopatadine Hydrochloride.
• Dual Action Mechanism:
  1. Selective $H_1$-Antihistamine: Competitively blocks histamine $H_1$ receptors on the ocular surface, delivering rapid relief (within minutes) from itching, redness, and burning.
  2. Mast Cell Stabilizer: Inhibits the degranulation of human conjunctival mast cells, preventing the release of histamine, prostaglandins, and leukotrienes when exposed to allergens.

Available Formulations & Standard Strengths

• 0.1% Concentration (e.g., Patanol): Dosed twice daily (1 drop every 6 to 8 hours).
• 0.2% Concentration (e.g., Pataday Once Daily Relief): Formulated for once-daily administration (1 drop every 24 hours).
• 0.7% Concentration (e.g., Pataday Extra Strength): Formulated for once-daily administration, providing relief for up to 24 hours for severe itch.
• Nasal Spray (e.g., Patanase / Olopatadine 0.6%): Indicated for the relief of symptoms of seasonal allergic rhinitis (nasal congestion, sneezing, runny nose).

Primary Clinical Indications

• Seasonal Allergic Conjunctivitis: Relieving ocular itching, tearing, and redness caused by airborne environmental allergens (pollen, ragweed, grass).
• Perennial Allergic Conjunctivitis: Long-term management of symptoms triggered by indoor allergens (dust mites, pet dander, mold spores).

Administration & Proper Technique

• Dosing Schedule: Use strictly as directed based on formulation strength (either once daily in the morning or twice daily spaced 6 to 8 hours apart).
• Instillation Steps:
  1. Wash hands with soap and water before handling the bottle.
  2. Tilt head back, pull down lower eyelid to form a small pocket, and squeeze 1 drop inside.
  3. Close eyes gently for 1 to 2 minutes without blinking excessively.
  4. Do not touch the dropper tip to your eye, eyelid, or hands to maintain sterility.
• Contact Lens Safety: Olopatadine drops contain Benzalkonium Chloride as a preservative, which can be absorbed by soft contact lenses.
  • Rule: Remove contact lenses before applying drops and wait at least 10 to 15 minutes before reinserting them. If eyes are red or irritated, avoid wearing contact lenses altogether.

Safety Profile & Common Side Effects

• Common Local Side Effects: Transient mild stinging or burning upon application, dry eyes, foreign body sensation, or mild headache.
• Systemic Side Effects: Very low systemic absorption; occasional mild pharyngitis or cold-like symptoms may occur.
