Betadrone-N (manufactured by Drugfield Pharmaceuticals) is a prescription combination ophthalmic/otic (eye and ear) drop and ointment. It combines a strong anti-inflammatory corticosteroid with a broad-spectrum aminoglycoside antibiotic to treat inflammatory conditions where a bacterial infection is present or suspected.

Active Ingredients & Dual Action

  1. Betamethasone Sodium Phosphate (0.1% w/v): A potent glucocorticoid corticosteroid.
    • Mechanism: Suppresses inflammatory responses, relieving ocular and otic redness, swelling, itching, heat, and pain.
  2. Neomycin Sulfate (0.5% w/v): A broad-spectrum aminoglycoside antibiotic.
    • Mechanism: Binds to bacterial ribosomes to inhibit protein synthesis, killing susceptible Gram-negative bacteria and Staphylococcus aureus.

Indications

Betadrone-N is indicated for short-term treatment of:
  • Ocular Conditions: Steroid-responsive allergic or inflammatory eye conditions (such as allergic conjunctivitis or anterior uveitis) accompanied by a bacterial infection or risk of bacterial infection.
  • Otic Conditions: Inflammatory conditions of the external ear canal (otitis externa) complicated by susceptible bacterial infections.

Administration & Recommended Use

  • Application: Instill 1 to 2 drops into the affected eye(s) or ear canal up to 3 or 4 times daily, or as directed by a physician.
  • Duration Limit ($\le 7$ Days): Should not be used for more than 7 consecutive days unless specifically advised and monitored by a healthcare professional. Prolonged use masks spreading infections and increases risk of side effects.
  • Hygiene: Do not touch the dropper tip to your eye, eyelid, inner ear, or fingers to avoid contaminating the solution.
  • Contact Lenses: Do not wear contact lenses while treating an eye infection or while using Betadrone-N drops.

Critical Warnings & Contraindications

Steroid-antibiotic drops carry significant precautions and should never be used as a general “red eye” fix without a medical exam:
  1. Strictly Contraindicated in Viral & Fungal Infections: Must never be used for viral infections (e.g., Herpes simplex keratitis / dendritic ulcers) or fungal/tuberculous eye diseases. Corticosteroids suppress local immunity, allowing viruses or fungi to rapidly multiply, which can cause severe corneal scarring or vision loss.
  2. Eardrum Perforation (Otic Caution): Do not use in the ear if the eardrum (tympanic membrane) is perforated. Neomycin is ototoxic and can damage inner ear structures if it enters the middle ear.
  3. Glaucoma & Elevated IOP: Contraindicated in patients with glaucoma. Prolonged steroid use in the eye can cause elevated intraocular pressure (IOP) and optic nerve damage.
  4. Neomycin Hypersensitivity: Local skin sensitization (allergic contact dermatitis) to Neomycin is common, causing redness, itching, and scaling around the eye or ear.
