Diamicron

SKU: RKMNFZ843
Brand: Rickmen Pharmaceuticals
Available Variations

Diamicron (developed by Servier) is an oral antihyperglycemic medication containing gliclazide, a second-generation sulfonylurea. It works primarily by binding selectively to sulfonylurea receptors (SUR1) on pancreatic beta-cell membranes. This blocks ATP-sensitive potassium channels, leading to cell depolarization, calcium influx, and exocytosis of pre-formed insulin into the bloodstream.

In addition to restoring early-phase insulin secretion in response to meals, Diamicron offers extra-pancreatic microvascular benefits by reducing platelet aggregation and adhesion, as well as increasing fibrinolytic activity on the vascular endothelium. Modern clinical formulations are typically available as Diamicron MR (Modified Release), which utilizes a hydrophilic matrix to release gliclazide continuously over 24 hours, enabling once-daily administration and providing a lower risk of hypoglycemia compared to older sulfonylureas.

Primary Medical Indications

Diamicron is indicated for adult patients (aged 18 years and older) for the treatment of metabolic conditions:

  • Type 2 Diabetes Mellitus: As an adjunct to diet, weight management, and exercise to lower elevated blood glucose levels when non-pharmacological interventions alone are insufficient.

  • Monotherapy: For non-obese or obese type 2 diabetic patients where metformin is contraindicated or poorly tolerated.

  • Dual & Triple Combination Therapy: Combined with other antidiabetic agents (e.g., metformin, DPP-4 inhibitors, SGLT2 inhibitors, or basal insulin) when monotherapy does not maintain target HbA1c levels.

Critical Safety Guidelines & Precautions

  • Hypoglycemia Risk: As an insulin secretagogue, Diamicron can cause hypoglycemia (low blood sugar), particularly if meals are skipped, delayed, or during unaccustomed physical exertion, excessive alcohol intake, or co-administration with other hypoglycemic agents.

  • Must Be Taken with Food: To prevent acute drops in blood glucose, Diamicron must always be taken immediately before or during a substantial meal containing carbohydrates.

  • G6PD Deficiency Warning: Treatment with sulfonylureas in patients with glucose-6-phosphate dehydrogenase (G6PD) deficiency can lead to acute hemolytic anemia. Consider alternative non-sulfonylurea agents in this population.

  • Severe Hepatic or Renal Impairment: Strictly contraindicated in severe renal or hepatic insufficiency, where the risk of prolonged and severe hypoglycemia is significantly elevated.

  • Contraindications: Contraindicated in patients with type 1 diabetes, diabetic ketoacidosis, diabetic pre-coma/coma, or hypersensitivity to sulfonylureas, sulfonamides, or related compounds.
