Dalacap is a potent lincosamide antibiotic containing clindamycin (as clindamycin hydrochloride). It works by binding reversibly to the 50S ribosomal subunit of susceptible microorganisms, suppressing bacterial protein synthesis. Depending on the drug concentration at the infection site and the sensitivity of the organism, Dalacap exerts either bacteriostatic or bactericidal action. It is particularly valued for its strong efficacy against Gram-positive aerobic bacteria and a wide range of obligate anaerobic bacteria.
Primary Medical Indications
Dalacap is indicated for adult and pediatric patients in the treatment of serious bacterial infections caused by susceptible anaerobic flora or Gram-positive pathogens:
-
Dental & Oral Infections: Severe dental abscesses, periodontitis, and refractory oral infections involving anaerobic bacteria.
-
Skin & Soft Tissue Infections: Severe acne vulgaris, cellulitis, erysipelas, furunculosis, impetigo, and post-surgical wound infections.
-
Bone & Joint Infections: Osteomyelitis and septic arthritis caused by Staphylococcus aureus.
-
Gynecological & Intra-Abdominal Infections: Pelvic Inflammatory Disease (PID), endometritis, tubo-ovarian abscesses, and peritonitis.
-
Lower Respiratory Tract Infections: Empyema, lung abscesses, and anaerobic pneumonia.

Critical Safety Guidelines & Precautions
-
Boxed Warning for C. difficile-Associated Diarrhea: Clindamycin therapy carries a well-established risk of Clostridioides difficile-associated diarrhea (CDAD), which can range from mild diarrhea to severe, potentially fatal pseudomembranous colitis. If significant diarrhea occurs during or after therapy, stop the drug and seek immediate medical evaluation.
-
Esophageal Irritation: Capsules should be swallowed whole with a full glass of water to reduce the risk of esophageal ulceration or irritation.
-
Penicillin-Allergic Alternative: Serves as a primary alternative for patients with severe hypersensitivity reactions (e.g., anaphylaxis) to penicillins or cephalosporins.
-
Neuromuscular Blockade Warning: Clindamycin has neuromuscular blocking properties that may enhance the action of other neuromuscular blocking agents; use with caution in patients receiving such drugs.
